    What You'll Do

    As a Clinical Team Lead (RN/PT) with Healthy Living at Home, you will retain your regular job responsibilities while assisting the DPCS in creating a team approach to the delivery of care to home health patients. The Clinical Team Lead will have a thorough understanding of the home health infrastructure and utilize such knowledge to maintain an organized, cohesive team environment.

    • Your coverage area will include: Santa Cruz 
    • Maintain regular job functions of patient care, documentation, and compliance associated within their respective clinical field
    • Participate in orientation and training of new hires
    • Serve as an advocate for the clinicians and patients of the home health agency
    • Organize and lead weekly case conferences to discuss patient status, including POC, barriers to progress, discharge planning, care coordination, and scheduling

    Who You Are

    • Graduate of nursing or physical therapy from an accredited organization and licensed to practice within the state.
    • One (1) year or more experience as a Registered Nurse or Physical Therapist
    • One (1) year or more of experience within the past five years in a home health agency, primary care clinic, or health facility
    • Knowledge and expertise in clinical decision-making and evaluation.
    • Complies with accepted professional standards and practice.
    • Demonstrates excellent observation, verbal and written communication skills.
    • Complies and maintains current CPR certification.
    • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
