    What You'll Do

    As an Intake Coordinator with Healthy Living at Home, you will manage all aspects of patient intake and scheduling. You will establish and maintain positive relationships with referrals and customers by responding to their requests and concerns in a timely manner.


    • Coordinate daily patient referrals and intake operations; completing all process steps for each referral received
    • Verify insurance eligibility, provide introduction calls to potential patients, follow up on quality assurance calls to ensure patient satisfaction, assist with Durable Medical Equipment, and input patient frequency and duration for clinicians
    • Ensure seamless transition of patients to home care by providing coordination of staff for start of care and evaluation processes

    Who You Are


    • Minimum of one (1) year of experience in health care (hospice, skilled nursing, MD office, hospital)
    • Ability to work alongside a team of professional and administrative personnel
    • Autonomous, organized, assertive, flexible, and cooperative in performing job responsibilities in a fast-paced work environment
