    What You'll Do

    As a Nurse Practitioner (NP) with Sojourn Hospice and Palliative Care, provide visits to our hospice patients and present findings to the Medical Director before re-certification is due.


    • Your coverage area will include: Merced County, Stanislaus County and San Joaquin County
    • Part-time opportunity
    • Provides visits to our hospice patients and present findings to the hospice Medical Director for Doctor’s re-certification approval
    • Provides a complete physical assessment and history of current and previous illnesses
    • Uses health assessment data to determine re-certification

    Who You Are

    • Holds unencumbered Nurse Practitioner (NP) license
    • Graduate of National League for Nursing (NLN) accredited school of nursing
    • Minimum of one (1) year of experience working under your NP license
    • Minimum of one (1) year of hospice experience
    • Possess and maintain a current CPR certification
    • A licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
    • Self-motivated and able to work with minimal supervision
